Job Description:

As a Case Manager with us, you will have the opportunity to be involved in the coordination of care and services for patients admitted to Saint Joseph Hospital / Intermountain Health who are identified as needing assistance or meeting Care Management criteria.

Scope

As a Social Work Case Manager, you need to know how to:

  • Coordinate the care and services for patients identified as needing assistance or meeting care management criteria.
  • Assess the patient’s prior level of functioning, access to and/or use of community resources and available support systems.
  • Identify appropriate admission and continued stay issues.
  • Attend, facilitate, and participate in rounds and case conferences.
  • Identify needs, facilitate, or provide education to physicians and ancillary departments/nursing units regarding case management, discharge planning process, and roles.
  • Participate in multidisciplinary groups and development of guidelines.

Minimum Qualifications:

  • Graduate of an accredited school of social work, sociology, or related field
  • Minimum of one (1) year of clinical experience

Preferred Qualifications:

  • At least two (2) years of case management experience in a hospital setting

Physical Requirements:

Interact with others by effectively communicating, both orally and in writing.- and -Operate computers and other office equipment requiring the ability to move fingers and hands.- and -See and read computer monitors and documents.- and -Remain sitting or standing for long periods of time to perform work on a computer, telephone, or other equipment.- and -May require lifting and transporting objects and office supplies, bending, kneeling and reaching.

$28.09 - $49.89
